I believe we ran nto a similar problem which was fixed when I upgraded OnTap and the Firmware

I've had similar problem in the past (new disk was not recognized at all - looked like an empty slot to the filer) and it was eventually tracked down to a bad LRC. If this is a clustered system, see if the other filer is seeing the disk.
