Just off the top of my head, are you sure that the offline ports are indeed attached to the port and switch you think they are? In the portshow 40 output, it says:

21:00:00:24:ff:73:20:73

Can you run the command nodefind 21:00:00:24:ff:73:20:73 for me, and tell me whether that's a WWN you've made an alias for, and what manufacturer it's from?
