I had just this problem and also was sent a second flash card. In my case
(to my shame!) I had the card in upside down!
The extender is keyed to fit only one way but the card can fit into the
extender either way. The instructions do not cover this and even NetApp
support had to go and look at a filer to see.
So, the card needs to go in with the label "NetApp
CompactFlash" upwards. In my case, putting it in upside down didn't
damage it.

one Filer to the 256MB without any problem. The other, I having
major problems. Twice already, Netapp sent me a new 256MB CF,
